   Community Outreach & Housing

Community Outreach & Housing has become a community leader in terms of understanding best practice concepts, including: use of generic housing dispersed widely in the community; provision of flexible individualized supports; consumer choice; assistance in locating and maintaining housing; no restrictions on length of time people can remain in the residence; and community based peer support.

Members define their own housing needs; individualized planning contributes to greater housing satisfaction, stability and longevity.

Members’ housing choices are not limited by previous classifications or identified disabilities; members have the right to try the most independent option they wish, with flexibility to try a variety of options and possibilities.

Connections is large enough that members can find compatible housing with partners and friends; there are strong linkages back to the Connections, where there is continuous support from other members and staff.

Connections works at making each person’s residence a home!

"This unit links peers and staff to people where they live.  They recognize 

the importance of a call or a place to drop in.  Members and staff link up with people so they don't have to be alone and this helps members prevent crisis 

        by connecting with help.  It's a role model for other 

               programs in the community."
